Are you an experienced and compassionate individual looking for a new challenge in the health and social care sector?

We have an exciting opportunity for a Care Coordinator to join our team and play a vital role in supporting our business with new care packages and overall care coordination.

This dynamic role offers a blend of office-based and community-based work. Your primary hours will be Monday to Friday, 8am to 4pm, but we're looking for someone who can be flexible. You'll need to support care delivery for new packages that may fall outside these hours, and there will be occasional weekend work as needed. 

As an ideal candidate, you'll hold a Level 3 Health and Social Care qualification. You must also be a driver with access to your own car for work purposes.

As part of our senior team, you'll contribute to our out-of-hours on-call rota, for which an enhanced weekly payment will be provided.

We're looking for someone with excellent interpersonal skills who thrives in a fast-paced environment where priorities can shift quickly. Strong IT skills are also essential, as you'll be working with an electronic care system.
